Application Form for instructions, requirements and completion. Valid Government-issued photo ID also required. Disability HOLO cards are available at the Transit Pass Office. Temporary Disability reduced fare ends when the Healthcare provider-prescribed timeframe
ends. Permanent Disability Card must be renewed every 4 years.
Disability Kamaaina – Proof of Residency Required.

Single Cash Fare $1.25
Fare is to be paid upon each boarding. Rider must present their Disability HOLO card to pay the reduced fare.
